- Different from outpatient Guarantee Letters, inpatient initial guarantee letters should only be made by the panel inpatient hospitals.
- Application of Initial Guarantee Letter (IGL) can be done through (1) Dashboard or (2) Patients.
- All IGL request will be processed within 1 hour
- Any change of details such as diagnosis and admission type after IGL approval would require the re-application of IGL
1. INITIAL GUARANTEE LETTER THROUGH DASHBOARD
1. Click > [+New GL].
2. Type in patient/member IC or passport number > [Search].
3. Click > [Request GL].
4. Download and print [Patient Consent Form] and [Attending Physicians Statement]. Patient/member are required to sign off the printed Patient Consent Form and admitting doctor to complete Attending Physician Statement before uploading into HealthMetrics Portal.
**Note: Both forms will be pre-filled with patient/members information.**
5. Complete the remaining mandatory information such as Admission no/MRN, Admission Type in
HealthMetrics Portal directly.
**Note: At this point of time, the keyed in information can be saved as draft and be completed later.**
6. After all information and documents are in order, click > [Next] to upload documents.
7. To review the request of IGL by clicking [Next], after uploading the necessary documents.
8. Click > [Submit for approval] after checking all information and documents.
Note: Information can be amended by click on [Edit].
2. INITIAL GUARANTEE LETTER APPLICATION THROUGH PATIENTS
1. Click on [Patients].
2. Follow Steps 2 to Step 8 as mentioned above
