REQUEST TOP UP & ADDITION OF DOCTOR(S)
- Top up of guarantee amount or addition of doctor(s) can be done at any point of time during admission.
- All requests should be submitted into the HealthMetrics portal directly with all necessary documents and forms.
- Top up amount and addition of doctor(s) can be requested through (1) Dashboard or (2) Admission Timeline tabs in the panel hospital module.
1. TOP UP REQUEST & ADDITION OF DOCTOR(S) THROUGH "DASHBOARD" > SECOND PANEL > "CURENT ADMISSIONS"
1. Look up the patient/member card in Dashboard by filtering the name and IC number. Click > [Actions] and click > [Request top-up & add-on].
2. Select the request type.
3. Download and print all necessary form(s) to be completed by attending doctors.
**Note: Should addition of doctor(s) is required, the new attending doctor will need to complete another “Attending Physician’s Statement” to be uploaded for review.**
4. After all information and documents are in order, click > [Next] to upload documents.
5. To review the request by clicking [Next], after uploading the necessary documents.
6. Click > [Submit for approval] after checking all information and documents.
**Note: Information can be amended by click >[Edit].**
7. Status of the request will be displayed in “Dashboard”, awaiting approval from HealthMetrics.
2.TOP UP REQUEST & ADDITION OF DOCTOR(S) THROUGH "ADMISSION TIMELINE"
1. In “Admission Timeline”, click on [Top-up/add-on] icon.
2. Follow Step 2 to Step 7 mentioned above.
