Government Hospital Admission Workflow (Cashless)

If you are going to get admitted to any government hospital, kindly follow the steps below:

Step 1: Download & fill up the forms available from this link Guarantee Letter Request Form for GH admission. 
Note: There will be a Guarantee Letter & Consent Form available from the link above. Kindly fill it up accordingly.
Refer to the list of Government Hospital via this link Portal Rasmi Kementerian Kesihatan Malaysia (moh.gov.my)

Step 2: Once the forms are all filled up, you will need to email the form to [e-mail] for a Guarantee Letter request.

Step 3: HealthMetrics will approve your Guarantee Letter within 2 hours. You will need to print it out, show it to the panel staff and pay the RM5 registration fee (mandatory).

Step 4: Proceed with your hospitalization/admission.

Step 5: You will be discharged upon recovery / doctor's approval.

Step 6: Kindly request your itemized billing before leaving the hospital.

Step 7: Government hospitals do not allow any limitations in terms of coverage & entitlement. Hence, everything is going to be covered except for your R&B upgrade.

Step 8: Collect all medications & MC (if any) and get well soon!

Important Notes:

1. The Government Hospital will not apply the Guarantee Letter for you.
2. Both Guarantee Letter & Consent Form is available from the link in Step 1. You will have to apply on your own or seek your next of kin to fill up both forms and email them to us - [e-mail].
3. Your hospitalization entitlement does not include an R&B upgrade. You will have to pay on your own.
