If you are going to get admitted in a hospital, kindly follow the steps below:
Important Notes:
1. Hospital may request you to place a minimal deposit in the range of RM300 to RM1000 for an admission.
2. The amount of deposit is different from each hospital.
3. The deposit may be refunded when hospital receives the Guarantee Letter from HealthMetrics or upon your discharge. (This is subject to the refund procedure of the hospital).
4. Hospital may request you to pay for any ineligible or uncovered expenses. Ineligible or uncovered expenses will be listed in your Final Guarantee Letter.
5. Please refer to your Insurance Policy for examples of ineligible or uncovered expenses.
Final Note:
1. Once you have been discharged from the hospital, you may submit the claims (where applicable) from the HealthMetrics Mobile App itself.
2. Submitting inpatient claims: From the Home page > Reimbursement claims > Submit and choose the claim type > Select treatment type > Fill in the Claim submission > Download the form and ensure all the documents uploaded are added a remark (*) as stated below > Submit.

Employees to add using a pen on each page of the document:

Berjaya Sompo Insurance, BSI: “For Berjaya Sompo Insurance claims purpose only”
The Pacific Insurance Berhad: “For The Pacific Insurance Berhad claims purpose only”
Allianz Life Insurance Malaysia Berhad: “For Allianz Life Insurance Malaysia Berhad claims purpose only”
Failure to do so, your insurance will reject the claim you have submitted.
