Additional Document Needed for AmMetLife Insurance Claim Only (Inpatient Benefits)

Members who are insured under AmMetLife are required to fill up an additional document upon submitting your claims. Follow the steps below:

1. Download and upload this Letter of Declaration for your AmMetLife insurance claim. Click here to retrieve the file.
   
2. Follow the normal process flow for submitting your inpatient claims. You may upload the additional document together with the Consent & Authorisation Forms field.
3. Proceed to upload the necessary documents and click on Submit.
   
   

   ❗Important Note: Employee to write at the top of each of the documents uploaded “For [insert insurance name] purposes only.” Failure to do so will result in the rejection of the claim submitted.

4. Your submitted claim will be reviewed and processed accordingly.
