Submitting a Reimbursement Claim (Inpatient Benefits)

As an insured member under HealthMetrics, you may be able to claim for your inpatient admissions including your pre & post hospitalisation visits.

1. From your HealthMetrics Employee App, add your payment account by uploading your bank details.
   
2. From the homepage, click on the Submit Claims icon > Fill up the Claim submission form > Choose Hospitalisation as the claim type.
   
3. Select the applicable treatment type for the claim > Fill up the Claim Submission form > Download the Hospital & Surgical Claim and ensure it is filled up and completed by your attending doctor.

4. If you are insured under any of the insurer(s) listed below, kindly proceed to download and upload the additional document required:
   i. Click here to retrieve the additional document required for AmMetLife.
5. Proceed to upload the necessary documents and click on Submit.
   

❗Important Note: Employee to write at the top of each of the documents uploaded “For [insert insurance name] purposes only.” Failure to do so will result in the rejection of the claim submitted.

5. Your submitted claim will be reviewed and processed accordingly.
