How do I reset password in the system if I forget my password?

 1: Go to www.healthmetrics.com

 2: Click on "SIGN IN" and choose healthcare partners

 3: Type in the assigned "Panel or clinic code", click on "Next"

 4: Type in username or email, click on "Next"

 5: Click on "Forgot password" 

 6: Type in again your "Panel code" and "Email address"

 7: Click on "Submit" 

 8: A password reset link will be sent to your email registered with HealthMetrics. 

 9: Check your email and reset password accordingly.
