NOTE: Panel specialist/hospital can only apply an outpatient GL for a patient on the actual appointment date/day itself and NOT pre-apply the GL in advanced as the system wouldn't allow it.
1. Login to HealthMetrics Portal
2. At [Dashboard], select [Outpatient Specialist]
3. You will be lead to Outpatient specialist [Dashboard]
4. Obtain the IC/ Passport from the patient, search the patient from HealthMetrics portal. Select the patient card and click on [Check-in] button] (Figure E)
5. Click on the [+Apply guarantee letter] button to apply for the patient (Figure F)
6. Next step of verification: Is the patient in the hospital now? Click on [Yes].
7. Input the visiting Specialist disciplines (mandatory).
8. Attach/Upload referral letter/appointment (if applicable).
9. Click on [Submit] (Figure G (i) & (ii))
10. Contact the phone number on the message box after your submission for expedition OR you can always refer to patient card for the contact person detail. Guarantee Letter will be approved soonest possible.
11. HealthMetrics portal will auto-refresh every 5 seconds, you can check the GL status without refreshing the page or an email will be sent to the person in charge upon GL approval. Once the pending Guarantee Letter is approved, the employee card will shift to the RIGHT side.
12. Print Guarantee Letter and Consent & Claim Form. Patient to sign the consent form before proceeding to send the patient for treatment. If Guarantee Letter being rejected, you may print out the GL rejection letter for both hospital and patient.
