To submit an outpatient claim, please follow the steps below:
1. Login HealthMetrics Portal
2. At [Dashboard], select [Outpatient Specialist]
3. You will be lead to Outpatient specialist [Dashboard]
4. Under the box named as [Pending Submission], look for the correct patient name with the correct GL number and visit date, then select [Submit now] / [Top up]
5. Key in required information under [Billing details]:
- Hospital bill reference no (Invoice number)
- Grand total bill amount (Total incurred charges for the visit)
- Attach detailed tax invoice
- Choose the correct attending doctor (If the doctor is not available in the selection, refer How do I add new doctor? )
- Upload Patient Consent Form and Completed Diagnosis Form (You may select [Upload later] if doctor is yet to complete the diagnosis form)
6. Select [Submit for review]
7. You may check the claim status in Outpatient Specialist [Dashboard], under Box [Submitted]
OR you may check in [Outpatient] > [Claims]
