You can apply outpatient guarantee letter (GL) via the HealthMetrics mobile app.
- Launch your HealthMetrics mobile app
- Go to the menu on top left corner
- Select Guarantee Letter
- Select Apply a new Guarantee Letter
- At Patient, select the correct individual (principal/dependent) who is visiting the doctor
- At Specialist discipline, select the correct specialist discipline
- Select the correct panel
- Select Next
- Select the appointment date if you have an appointment date. Otherwise, select Next
- Upload supporting documents (Referral letter/appointment card/prescription note) **Note: If supporting document is not required as per company policy, employee can simply upload a random photo/document allow the next process to go through**
- Select Next
- Read the authorisation & indemnity section
- Tick if you agree to the terms and conditions of the authorisation and indemnity section above
- Select Review to go through employee's application again. Make sure that everything selected is correct
- Check the box: I agree that the information provided is correct and accurate
- Check the box: I have attached clear shot of the referral letter or appointment card
- Select Submit
