As an insured member under HealthMetrics, you may be able to claim for your inpatient admissions including your pre & post hospitalisation visits.
- From your HealthMetrics Employee App, add your payment account by uploading your bank details.
- From the homepage, click on the Submit Claims icon > Fill up the Claim submission form > Choose Hospitalisation as the claim type.
- Select the applicable treatment type for the claim > Fill up the Claim Submission form > Download the Hospital & Surgical Claim and ensure it is filled up and completed by your attending doctor.

- Proceed to upload the necessary documents and click on Submit.
❗Important Note: Employee to write at the top of each of the documents uploaded “For [insert insurance name] purposes only.” Failure to do so will result in the rejection of the claim submitted.
